Join us for an informative presentation with Lisa Zak from VITAS Healthcare as she guides us through the essentials of hospice care. This program will provide a clear understanding of what hospice is, who it serves, and the benefits it offers to patients and families. In addition, Lisa will introduce the Five Wishes, a nationally recognized advance care planning tool that helps individuals express their personal, emotional, and spiritual needs alongside medical preferences. This session is designed to empower attendees with knowledge and resources to make informed decisions for themselves and their loved ones.
